= Your Last Name

= Your full phone
number, including area code. (Please be sure to enter this without any
dashes, parentheses, dots, spaces, or any other punctuation.)
Please
make sure that this information is contained on one line only
– if the text wraps over a second line, your request may not
be processed.
Sending
this e-mail will block all calls from our PBX to
your phone number. If
you are on our "Do Not Call" list, you will not be able to receive
calls from Aplus.Net. You
will, however, be able to call any Aplus.Net number to reach support or
sales personnel.
Important:
Even if your phone number is in our "Do Not Call" list, it is still
possible that you receive a phone call from one of our Customer Care
Representatives. Such calls are very important since they could be
related to account verification, plan renewal, etc.
